📝 Guest Permission forms are paper copies located in the OSM and submitted by February 4th.
Dance Information
- To be admitted to the dance, all guests must present their current school or State Photo ID/Driver’s License and their ticket.
- Students (with their school IDs) may arrive between 5:30 and 6:45 pm.
- Students will not be allowed into the dance after 7:00 pm and students may leave after 8:00 pm
- Dancing will begin at 6:00 pm and continue until 9:00 pm. Students/guests must leave at 9:00 pm
Permission Slips
Permission slips for Winter Formal are now available ONLINE at www.indiantrailhawks.com under the Registration tab. SIGN UP TODAY!
Guest permission slips are available in the OSM for pick up. You will be required to return the completed guest permission slip to the OSM with a copy of your guests ID. Graduated guests over the age of 18 must ALSO submit a Criminal Background Request online prior to 2/4/26. Make sure you read all the eligibility guidelines!
Payment & Ticket Information
Ticket payments will be available on Infinite Campus. All tickets – ITHSA Student and Guest – must be made on Infinite Campus before you can pick up your ticket. A link to purchase tickets will be sent out with your eligibility status.
Refunds will not be issued to students who purchase tickets but do not meet the eligibility requirements!
Eligibility Requirements
Students attending Winter Formal 2025-2026:
- May not have any truancies on or after January 19, 2026
- May not have more than 5 unexcused tardies on or after January 19, 2026
- May not have any OSS or ISS (including .5 day) suspensions on or after January 19, 2026
- All detentions must be served by February 12th, 2026
No Refunds will be given to students who purchase a ticket and accumulate an ISS, OSS, or fail to serve their detention in the time frame stated above.
Additional Information
Security will be checking students (including hand held metal detectors) at the door and security will be in attendance for the duration of the dance.
- Students who are suspected of being under the influence may be given a field sobriety test, barred entrance into the dance, have parents notified and face school discipline. (NO REFUNDS WILL BE GIVEN)
- Student behaviors that are deemed as inappropriate or unsafe by administration will result in disciplinary action which may include removal from the dance. Parents/guardians of students who engage in prohibited behaviors will be contacted and students will be sent home. If school officials are unable to contact a guardian, student violators will not be permitted to remain in the dance area, but will be held in a detention area. (NO REFUNDS WILL BE GIVEN)
- Students may not wear items that display offensive symbols, inappropriate language, items that can be considered gang related, outfits that expose inappropriate amounts of skin, or dresses that are not long enough. School administration reserves the right to define inappropriate.
- Any dance attendee who violates these rules while in attendance at the dance may be subject to disciplinary action.
